- History of Flight:
On September 3, 2026, at about 0932 local time, a Bell OH-58A Kiowa, N480FT, registered to Dillon Flight Test Inc, sustained substantial damage when it was involved in an accident near Phoenix, Arizona. The two pilots were not injured.
The FAA reported that the helicopter landed hard while practicing auto rotations.
