Waco RNF, N110Y, and Cirrus SR20 G6, N506AV, incident occurred on December 14, 2025, at Phoenix-Deer Valley Airport (DVT/KDVT), Phoenix, Arizona

- History of Flight:
On December 14, at about 0907 local time, a privately-registered Waco RNF, N110Y, and Cirrus SR20 G6, N506AV, registered to Westwind School of Aeronautics Phoenix LLC, doing business as United Aviate Academy, were involved in a ground collision during taxi operations at Phoenix-Deer Valley Airport (DVT/KDVT), Phoenix, Arizona. The sole pilot onboard the Waco and the pilot and passenger onboard the Cirrus were not injured. Botrh aircraft sustained unknown damage. The Waco was operated as a Title 14 Code of Federal Regulations Part 91 flight. The Cirrus was being operated as a Title 14 Code of Federal Regulations (CFR) Part 91 instructional flight.

According to the FAA, the Waco crashed into the rear of the Cirrus, and the time of the event was reported as 16:07:00Z. According to ADS-B data, the Cirrus landed at KDVT at 16:00Z and proceeded to the ramp via taxiway A6. The last ADS-B data point was recorded at 16:05:49Z. At about 17:35Z, the airplane was moved from the north ramp and across the runway towards the south area of the airport.

On December 3, 2025, about 1600 central standard time, a Mooney Aircraft Corp. M20K, N57229, was substantially damaged when it was involved in an accident near Foley, Alabama. The pilot sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to the pilot, the flight departed from Holk Field at Foley Municipal Airport (5R4), Foley, Alabama, with 54 gallons of fuel. The pilot flew to the north at an altitude 4,500 ft mean sea level and conducted several practice maneuvers, including steep turns, before returning to the airport about 1 hour later. The pilot switched the fuel selector tank position twice (every 30 minutes) based on his Garmin timer and he switched the fuel selector position a third time on his way back to 5R4. Shortly thereafter, the airplane experienced a total loss of engine power. The airplane was too far north to reach the departure airport; the pilot diverted to Bay Minette Municipal Airport (1R8), Bay Minette, Alabama, set best glide speed (80 kts) and attempted to restart the engine by completing the “engine out” checklist. The pilot’s attempts to restore engine power were unsuccessful.

The airplane was unable to glide to 1R8, and the pilot performed an off-field landing to a farm pasture. During landing, the airplane impacted a barbed wire fence, which sheared the landing gear; it then landed on rough terrain where it skidded to a stop. The airplane’s wings were buckled and torn in multiple locations, and there was damage to the wing spar and underside of the fuselage. The two-bladed metal propeller exhibited no rotational damage.

An FAA inspector recovered an engine data monitor that was sent to the NTSB recorders laboratory for download. The wreckage was retained for further examination.

The FAA reported that: "Aircraft experienced a small fire in the engine area after taxiing to the ramp and fire was extinguished."

On December 13, at about 1341 local time, a Bell 505 Jet Ranger X helicopter, N865S, registered to B&H Equipment Rental LLC out of Missoula, Montana, sustained substantial damage when it was involved in an accident near Dilley, Texas. The pilot sustained serious injuries, and passenger sustained minor injuries.

The FAA reported: "Aircraft experienced a hard landing after takeoff in a field and came to rest on its side." ADS-B data shows that the helicopter departed from a field and climbed to about 25 ft before initiating a descent and disappearing from coverage.

ADS-B data shows an uneventful approach to runway 26 before the incident. It is unclear if this was a single main/nose gear-up landing or an all gear-up landing.

- History of Flight:
On December 13, 2025, at about 1220 local time, United Airlines flight UA803, Boeing 777-224ER, N78004, sustained unknown damage when it was involved in an incident at Dulles International Airport (IAD/KIAD), Dulles, Virginia. The two pilots, five cabin crew, and 290 passengers were not injured. The international scheduled passenger flight originated from KIAD, and was destined to the Haneda Airport (HND/RJTT), 
Tokyo, Japan.

According to the FAA: "Aircraft no. 1 engine and engine cowling separated and caught fire on departure, returned to IAD." However, local news reported that no parts departed the aircraft, and that the incident caused a grass fire on airport property.
