On September 18, 2025, about 1320 central daylight time, a Piper PA-32R-301 airplane, N318JM, was substantially damaged when it was involved in an accident near Oxford, Mississippi. The pilot and passenger sustained serious injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

According to witnesses, the airplane pitched up aggressively at full engine power during rotation from the runway and continued to hold the nose-high pitch attitude, remaining at full engine power, until the airplane entered an aerodynamic stall and the right wing dropped. The airplane impacted terrain on the right side of the runway and came to rest upright against trees on airport property resulting in substantial damage to both wings, the fuselage, and the stabilator.

According to the airport manager, the airplane’s right-wing fuel tank was topped off with 38.9 gallons of 100 Low-Lead aviation gasoline before the flight departed. Postaccident fuel samples were taken from both the airplane and the fuel distribution truck with no anomalies found. The fuel selector valve was found with the right tank selected.

The airplane was retained for further examination.

On August 12, 2025, about 1330 Pacific daylight time, Cessna TR182 airplane, N619MW, was substantially damaged when it was involved in an accident near San Diego, California. The pilot and passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that shortly after reaching the top of his climb he noticed that the airplane was not accelerating. He cancelled his instrument flight plan and decided to return to Montgomery-Gibbs Executive Airport (MYF), San Diego, California, where he had departed from moments earlier. During the descent, the landing gear was lowered and there was no positive gear down indication. The pilot then declared an emergency with air traffic control. He contacted the tower at MYF and performed two low-pass fly-bys at which time the tower controller noted that the nose landing gear was extended and the main landing gear was retracted. The pilot made multiple attempts to lower the landing gear without success including the use of the emergency gear extension handle. During landing, the airplane sustained substantial damage to the left horizontal stabilizer.

According to the FAA, local mechanics discovered that the airplane’s hydraulic system was devoid of hydraulic fluid. In addition, they found the landing gear down position hydraulic fluid pressure hose had failed at the swedge near the fitting end.

The wreckage was relocated to a secure facility for further examination.

Piper PA-28-140 Cherokee E, N4218T, accident occurred on October 5, 2025, near Isedor Iverson Airport (HZX/KHZX), McGregor, Minnesota

On October 5, 2025, about 1310 central daylight time, a Piper PA-28-140, N4218T was substantially damaged when it was involved in an accident near McGregor, Minnesota. The pilot and two passengers were seriously injured. The airplane was operated under the provisions of Title 14 Code of Federal Regulations Part 91 as a personal flight.

The airplane departed from Isedor Iverson Airport (HZX) McGregor, Minnesota, and impacted a marsh field about .6 nm from the departure end of runway 14. According to law enforcement, the pilot said that the engine lost power and the airplane would not climb.

A postaccident inspection revealed that the fuel selector was set to the right fuel tank and there was fuel in both wing fuel tnaks. Fuel samples were taken from both fuel tanks, and no water was present in either sample.

The wings, fuselage, and empennage were substantially damaged. The airplane was retained for further examination.
