Beechcraft V35B Bonanza, N179R, incident occurred on October 2, 2025, at Black River Falls Municipal Airport (BCK/KBCK), Black River Falls, Wisconsin

- History of Flight:
On October 2, 2025, at about 1438 local time, a privately-registered Beechcraft V35B Bonanza, N179R, sustained unknown damage when it landed gearup on runway 8/26 at Black River Falls Municipal Airport (BCK/KBCK), Black River Falls, Wisconsin. The sole pilot onboard was not injured. The flight originated from La Crosse Regional Airport (LSE/KLSE), 
La Crosse, Wisconsin, at 1317 LT, and was destined to an unknown location.

According to preliminary ADS-B data, the airplane entered a left turn after departure from KLSE and proceeded to the north. At 1326 LT, the airplane entered a series of manoeuvres and then climbed to an altitude of 3,100 ft. At 1340 LT, the airplane entered yet another series of turns and manoeuvres in the vicinity of KBCK and continued to climb up to an altitude of 5,800 ft before it entered a descent towards the airport (figure 1). The last ADS-B return was recorded 3.10 miles west of KBCK.

Cirrus SR22 G2, N812SE, accident occurred on October 2, 2025, near Lincoln, Alabama

- History of Flight:
On October 2, 2025, at about 1126 local time, a Cirrus SR22 G2, N812SE, registered to Pegasus LLC out of Birmingham, sustained substantial damage following a forced landing near Lincoln, Alabama. The pilot and two passengers were not injured. The flight originated from the Birmingham-Shuttlesworth International Airport (BHM/KBHM), 
Birmingham, AL, at about 1107, and was destined to an unconfirmed location.

According to preliminary ADS-B data, the airplane entered a climbing left turn to an inflight cruising altitude of 3,200 ft after departure from BHM. About 13 minutes into the flight, the airplane started a rapid descent about 12 miles west of Talladega Municipal Airport (KASN), Talladega, AL. The airplane appeared to be descending towards the KASN, but it eventually landed in a field about 3.60 miles west of it (figure 1).

The Cirrus Airframe Parachute System (CAPS) was not used. The airplane landed in an open field and sustained damage to the right wing and fuselage. The airplane was powered by a Continental IO-550-N engine. All three propeller blades remained attached to the propeller hub. One propeller remained undamaged, the second one sustained minor damage, and the third one was broken about halfway through.

Winds at the time were reported from 080° at 11 knots.

The pilot reported that he was landing on runway 36L. The airplane in front of him landed short of their assigned point and started to decelerate rapidly. The pilot wanted to avoid a collision; however, he applied the brakes too hard and the airplane ground looped. The right wing and right elevator struck the ground which resulted in substantial damage. The pilot stated that there were no mechanical anomalies with the airplane that would have precluded normal operations. 

- Probable Cause: The pilot’s aggressive brake application which resulted in a loss of directional control and a ground loop.

The pilot was flying in the Homebuilt Showcase during the EAA AirVenture air show. The pilot reported that while landing with a variable tailwind the airplane touched down about 2,500 ft down the runway, bounced, and landed hard. The pilot applied brakes, and the right main tire began to squeal. The airplane departed the landing surface to the left. The right main landing gear collapsed, the nose landing gear separated. The airplane sustained substantial damage to the engine mounts and the right wing. The pilot reported that there were no mechanical malfunctions or failures that would have precluded normal operations. 

- Probable Cause: The pilot’s failure to maintain control of the airplane while landing with a tailwind.

On September 11, 2025, about 0905 Pacific daylight time, an experimental amateur-built Zenith CH801, N801CS, was substantially damaged when it was involved in an accident near Jackpot, Navada. The pilot and three passengers were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that the airplane had just been purchased, and the planned itinerary was to depart Grants Pass Airport (3S8), Grants Pass, Oregon, to their final destination of Wichita, Kansas with multiple fuel stops.

After conducting a planned rest stop at Jackpot Airport/Hayden Field (06U), Jackpot, Nevada, he conducted a preflight inspection of the airplane, which included verifying fuel levels with the use of a dip stick. The pilot taxied to runway 33, where he performed an uneventful engine runup. During takeoff on runway 33, the engine initially developed full power of 4,500 rpm. When the airplane was approximately two miles northwest of 06U, the engine rpm decayed to about 3,700 rpm. Subsequently, the airplane could not maintain altitude over rising terrain and the pilot initiated a forced landing to an open field with sparse vegetation. During the landing roll, the left main landing gear collapsed, and the left wing impacted the surface, which resulted in substantial damage to the left wing.

The airplane has been secured for examination.

On August 31, 2025, about 1100 Pacific daylight time, an American Champion 8KCAB Super Decathlon 177, N3357T, sustained substantial damage when it was involved in an accident near Jamesburg, California. The pilot and pilot rated passenger sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that they were on a scenic flight, maneuvering over a canyon area between 1,500 to 2,000 ft above ground level (agl) with clear skies and no turbulence. With the pilot-rated passenger on the controls, she leveled off about 2,000 ft agl, and then commented that the airplane “wanted to keep descending.” As directed by the pilot, she advanced the throttle to full power, but the airplane continued to descend. She maintained the airplane’s heading while the pilot manipulated the throttle, but there was no response from the engine and the airplane continued to descend and lose airspeed. The pilot then initiated a precautionary landing onto rough terrain where the airplane struck a tree and came to rest on sloped terrain. The airplane sustained substantial damage to both wings and fuselage.

The airplane was recovered to a secure facility for further examination.

On August 14, 2025, about 1915 easter daylight time, a Piper-PA12, N4014M, was substantially damaged when it was involved in an accident near Pembroke Pines, Florida. The private pilot sustained serious injuries, and the passenger received a minor injury. The airplane was operated as a title 14 Code of Federal Regulations Part 91 instructional flight.

The pilot and passenger reported that they were preparing to take off on runway 19. According to the pilot, the wind was from 240° at 5 knots. After the application of full power, the airplane began rolling down the runway. The tail lifted off the ground and the airplane immediately yawed to the left. Application of full right rudder was ineffective in correcting the yaw and the airplane departed the runway surface.

The pilot reduced power but was unable to stop the airplane. The airplane impacted a sloped concrete and dirt berm, went airborne, then impacted an airport perimeter fence before coming to rest. The resulting impact resulted in substantial damage to the airframe and wings; the wing spar was broken, and the engine compartment was crushed and displaced aft.

The wreckage was retained for further examination.

Bombardier CRJ-900LR, N902XJ, and Bombardier CRJ-900LR, N480PX, both owned by Delta Connection and being operated by Endeavor Air, accident occurred on October 1, 2025, at New York-La Guardia Airport (LGA/KLGA), New York, New York

- History of Flight:
On October 1, 2025, at about 2156 local time, Delta Airlines flight DL5047, a Bombardier CRJ-900LR, N480PX, and 
Delta Airlines flight DL5155, a Bombardier CRJ-900LR, N902XJ, were both substantially damaged following a ground collision at New York-La Guardia Airport (LGA/KLGA), New York, New York. At least one passenger sustained injuries. Both airplanes were operated under Title 14 Code of Federal Regulations Part 121 as regularly scheduled domestic passenger flights. DL5047 was coming in from Charlotte-Douglas International Airport (CLT/KCLT), Charlotte, North Carolina, and DL5155 was taxiing for departure, destined to Roanoke-Blacksburg Regional Airport (ROA/KROA), Roanoke, Virginia.

The FAA reported that; "N480PX: aircraft was struck by EDV5155 near taxiway A and taxiway M damaging the nose cone and windshield, New York, NY.

N902XJ: aircraft struck EDV5047 near taxiway A and taxiway M damaging the right wing, New York, NY."

Preliminary ADS-B data shows that, at 2153 LT, DL5047 landed on runway 4 and proceeded to taxiway G and then taxiway B. At 2153, DL5155 proceeded from the parking spot towards taxiway A. At about 2155, DL5155 was on the taxiway, and at the same, DL5047 was on taxiway B. At 2156:26, DL5047 began entering taxiway M with a reported groundspeed of 9 knots. At the same time, DL5155 was roughly 350 ft from the intersection. At 2156:36, DL5047 was on the intersection with a reported groundspeed of 8 knots, and so was DL5155, with a reported groundspeed of 21 knots. It appears the collision occurred within the 2156:39 - 2156:41 timeframe, because DL5047 came to a stop and DL5155 began slowing down and veered a little bit to the left. DL5047 was moved from the accident spot by 2214 LT. DL5155 was moved by 2358 LT.

Figure one shows a drawn out path for both flights (red is DL5047, blue is DL5155) on the airport diagram. Figure 2 shows a side by side comparison of both airplanes as seen on ADS-B exchange, and figure 3 shows the same track on Google Earth. The reported weather at KLGA, at 2151 (about 5 minutes before the accident) included: winds 020° at 10 knots, 10 statute miles visibility, no clouds under 12,000 ft (CLR), a temperature of 17° C, a dew point of 2° C, and a barometric altimeter setting of 30.27 inches of mercury.
