On July 17, 2026, at 1748 central daylight time, an experimental amateur-built Vans Aircraft Inc RV-8A, N22SZ, sustained substantial damage when it was involved in an accident in Mineral Point, Wisconsin. The commercial pilot was uninjured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. 

The pilot stated that while en route to Oshkosh, Wisconsin, he stopped at Iowa County Airport, Mineral Point (MRJ), Wisconsin, for fuel and to wait for visual flight conditions to improve at the destination airport. He then departed MRJ, and after climbing to 9,000 ft mean sea level, he switched fuel tanks and a “moment or so” later, the engine lost power. He then initiated a return to MRJ, during which he attempted several engine restarts. The engine restarted during the descent to MRJ, then lost power, restarted again, and continued to operate for the remainder flight to and taxi at MRJ. After the flight, the pilot stated that he talked to six or eight pilots at MRJ, two of which were mechanics, and a mechanic based at MRJ, about the loss of engine power that he had experienced. The group discussed mechanical and electrical fuel pump failures, carburetor icing, vapor lock, and test flying the airplane. 

On the following day, the pilot attempted a test flight of the airplane from MRJ. The pilot stated that the engine start for the test flight was “very clean normal start,” there were no hot start or fuel vapor problems, and the engine run-up was normal. He said that shortly after takeoff and early in the climb, the airplane engine began to run rough, and he decided to end the test flight and return to the airport. While turning left and climbing into the airport traffic pattern, the engine lost power. The pilot then decided to turn and land the airplane in the opposite direction since there was no engine power to fly an airport traffic pattern. During the turn, the engine started again but later lost power, and the airplane was high and fast. The pilot performed a forced landing on runway 11 where it impacted the runway surface. The airplane sustained substantial to both wings, the engine firewall, and an engine mount.

The airplane was retained for further examination.

On July 20, 2026, at 0643 central daylight time, a Piper J3C-65 airplane, N92432, sustained substantial damage when it was involved in an accident near Richmond, Texas. The commercial pilot sustained serious injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. 

The airplane impacted the roof of a house near the airport traffic pattern for Covey Trails Airport, Richmond, Texas. The airplane sustained substantial damage to the fuselage and wings. The pilot stated that he did not remember the circumstances leading to and nature of the accident. 

Postaccident examination of the airplane confirmed flight control continuity from the cockpit flight controls to the respective flight control surfaces. The two-blade wood propeller and metal propeller spinner did not display torsional features consistent with engine power. 

The airplane was retained for further examination.

On July 10, 2026, about 1023 mountain daylight time, an experimental amateur-built Vans Aircraft Inc RV-12 airplane, N812MM, sustained substantial damage when it was involved in an accident near Grand Junction, Colorado. The pilot and a passenger were uninjured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. 

The pilot stated that he picked up the airplane at Blake Field Airport (AJZ), Delta, Colorado, after a condition inspection was completed and planned to fly to Mineral County Memorial Airport, Creede, Colorado. After departure from AJZ, the Garmin G3X Touch display dimmed and shut down, which was immediately followed by a total loss of engine power. The pilot stated there was no electrical power to the starter switch, but the communication radio continued for function. The pilot then performed a forced landing to a field during which the airplane sustained substantial damage to the horizontal stabilizer. 

The airplane was retained for further examination.

On July 23, 2026, at 1440 eastern daylight time, an American AA-1, N6132L, was substantially damaged when it was involved in an accident near Kearneysville, West Virginia. The pilot and pilot-rated passenger were seriously injured. The airplane was being operated as a Title 14 Code of Federal Regulations Part 91 personal flight. 

A review of ADS-B data showed that the airplane departed from Eastern West Virginia Regional Airport (MRB), Martinsburg, West Virginia at 1345 and proceed about 20nm south where it performed a series of maneuvers before flying back toward MRB. During a postaccident interview, the pilot stated that while he was descending the airplane from 3,000 ft to 1,500 ft to enter the airport traffic pattern, the engine suddenly lost all power. Unable to reach the airport, he elected to make an emergency landing in a field about 5 nm south of MRB. During the forced landing, the airplane’s right wing and engine separated from the fuselage. The fuselage, left wing, and empennage were substantially damaged. 

The wreckage was recovered and retained for further examination.

On July 7, 2026 about 1430 Alaska daylight time, a Piper PA-18-150 airplane, N824JT, was substantially damaged when it was involved in an accident near Mekoryuk, Alaska. The pilot and one passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 135, on-demand charter flight. 

The airplane, owned and operated by Renfro’s Alaskan Adventures, departed from the Mekoryuk airport (MYU) at 1321 to conduct aerial game surveys for the Alaska Department of Fish and Game. About 1 hour and 10 minutes into the flight, about 200 ft. above the ground (agl) the engine began to run rough. The pilot applied carburetor heat, switched the fuel selector to both and began using different power settings. The engine initially began to run normally but subsequently loss partial power. The pilot elected to make a precautionary landing to the tundra, but while on final approach, all engine power was lost. The airplane touched down wings level and a slight nose up pitch. The landing gear separated from the fuselage and the airplane slid to a stop. The airplane sustained substantial damage to the fuselage and wings.

A wreckage examination is pending recovery of the wreckage.

On July 20, 2026, at 1030 eastern daylight time, a Cessna 152 airplane was substantially damaged when it was involved in an accident near Pell City, Alabama. The flight instructor and student pilot were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight. 

According to the instructor they departed St. Clair County Airport (KPLR), Pell City, Alabama, with full fuel tanks for a training flight. About four minutes after departure about 1,500 to 2,000 ft above ground level (agl), the engine lost all power and the instructor performed a forced landing to a field. After landing the airplane continued through a fence and the left wing impacted a tree. 

According to a Federal Aviation Administration inspector that traveled to the scene the left fuel tank was ruptured. The airplane was recovered and an engine test run was performed. The engine did not run smoothly, and the throttle had to be pumped to keep the engine running. 

The engine was shut down and secured for further examination.
