- History of Flight:
On December 15, 2025, at about 0900 local time, a Bell 407, N404LL, registered to Rocky Mountain Holdings LLC out of Greenwood Village, Colorado, and being operated by TriState CareFlight onbehalf of Air Methods Inc, sustained substantial damage when it was involved in an accident in/near Las Vegas, Nevada. The pilot and three passengers were not injured. The helicopter was being operated as a Title 14 Code of Federal Regulations Part 135 air medical flight.
According to the FAA: "Aircraft on post flight inspection and damage was found to the tail rotor." Flight-tracking history show that in the early morning hours of December 15, the helicopter arrived at the Sunrise Medical Center following a flight from Bullhead City, Arizona. The following day, the helicopter was flown into the Las Vegas-Harry Reid International Airport (LAS/KLAS), Las Vegas, Nevada, before being ferried back to Kingman, Arizona. It appears the helicopter was engaged in very short test flights after returning to Kingman (see figure).
