On June 17, 2024, at 1949 eastern daylight time, a Lockheed 12A airplane, N2072, was substantially damaged when it was involved in an accident near Jackson, Georgia. The commercial pilot and two passengers were seriously injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot stated that, 10 days before the accident, the tailwheel-equipped airplane underwent maintenance on the right main landing gear strut, which required disconnection and subsequent reinstallation of the right brake line. Following the maintenance of the strut, the brake system was serviced and no anomalies or leaks were observed. 

The pilot reported that the preflight inspection, engine run-up, and flight were uneventful; however, during the landing roll, the right brake pedal became ineffective. The airplane then veered left, departed the runway, and subsequently impacted a tree. The pilot and two passengers were seriously injured and the airplane was substantially damaged. A postaccident examination of the airplane revealed that the right brake line fitting had loosened near a section of the landing gear assembly that flexed during gear movement. The brake line B-nut fitting was found finger-tight, and hydraulic fluid residue was observed on and around the B-nut fitting mount, consistent with hydraulic fluid leakage. 

Because the airplane required differential braking for directional control during ground operations, the ineffective right brake limited the pilot’s ability to maintain directional control, resulting in a runway excursion during the landing roll. 

- Probable Cause: Maintenance personnel’s failure to properly secure the right brake line, which resulted in a loss of right brake effectiveness during the landing roll and a subsequent runway excursion.

The accident flight was the non-certificated pilot's first flight in the airplane, which the pilot had recently purchased and taken possession of on the day of the accident. The airplane was on a steep approach as the pilot attempted to land and hit an approach light short of the runway threshold. The airplane then bounced and struck another set of lights before landing hard on the runway. Subsequently, the airplane came to rest on the runway and a postimpact fire ensued. A witness who arrived at the scene of the accident stated that the pilot told him that he had misjudged the landing. 

A witness reported that the pilot conducted a steep approach to land, consistent with an unstable approach. The airplane was not available for examination following the accident, and whether a mechanical anomaly contributed to the accident could not be determined. The investigation was unable to determine the extent of the non-certificated pilot’s previous flight experience or whether the pilot had received any formal or informal flight instruction.

- Probable Cause: The non-certificated pilot’s unstable approach to landing, which resulted in a collision with approach lighting, hard landing, and postimpact fire.

- History of the Flight:

On May 21, 2024, about 1215 Pacific daylight time, an Engineering & Research Ercoupe 415-C, N99577, sustained substantial damage when it was involved in an accident near Ontario, Oregon. The pilot sustained serious injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. 

A certified flight instructor who was in an airplane holding short of runway 15 reported that the airplane was on a steep approach as it attempted to land and struck an approach light short of the runway threshold. The airplane then bounced and struck another set of lights before landing hard on the runway. Subsequently, the airplane came to rest on the runway, and a postimpact fire ensued. The airplane sustained substantial thermal damage to the fuselage, and the pilot sustained serious burn injuries. Another witness who arrived at the scene of the accident stated that the pilot told him that he had misjudged the landing. Subsequently, the airplane was destroyed in a postaccident fire. 

The wreckage was removed from the runway but was disposed of before the airframe and engine could be examined by the FAA or NTSB. 

The pilot was not a certificated pilot and did not hold a current FAA medical certificate, nor did he have a current medical; the FAA reported that he was denied a medical certificate about six years before the accident. 

Multiple attempts were made to contact the pilot; however, those attempts were unsuccessful while the pilot was recovering in a burn unit and under anesthesia. The pilot did not provide a statement to the FAA or NTSB after he was released from the hospital, nor did the pilot submit a National Transportation Safety Board Pilot/Operator Aircraft Accident/Incident Report Form 6120.1.

On May 30, 2026, about 0744 Atlantic standard time, a Cessna T210, N210PL, was substantially damaged when it was involved in an accident near Loiza, Puerto Rico. The pilot and passenger were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

The pilot reported that the accident flight was the first flight post maintenance. During takeoff and the initial climb, the engine was running normally with no discrepancies observed. After 5 minutes of flying, at about 3,000 ft mean sea level (msl), the pilot noticed an engine vibration. He then noted that after about 1 minute, the engine began to produce “explosion” noises, the oil pressure was very low, and the oil temperature was high. The pilot maneuvered to return to the departure airport and declared an emergency; however, the airplane would not have been able to reach the airport, and he performed a forced landing on a road. As the airplane touched down, it contacted a pole and quickly decelerated. 

Postaccident examination revealed that the airplane sustained substantial damage to the right wing, fuselage, and empennage. An FAA inspector found several gallons of fuel in both fuel tanks.

The wreckage was retained for further examination.

On June 2, 2026, at 0945 eastern daylight time, a Cirrus SF50, N129KA, powered by a Williams International FJ33-5A turbofan engine, experienced an uncontained engine failure during climb from Greene County Regional Airport (KCPP), Greensboro, Georgia. There were no injuries to the pilot or passenger. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 general aviation flight from KCPP destined for Sgt. Lee Russell Carroll County Airport (KHZD), Huntingdon, Tennessee.

According to the pilot, after takeoff at about 8,000ft above mean sea level (MSL), he received an engine inter-turbine temperature (ITT) warning and full authority digital engine control (FADEC) NO DISPATCH cockpit alerting system (CAS) message. He then contacted air traffic control (ATC) and requested a hold at 10,000 ft MSL, and in accordance with the checklist procedures, reduced the thrust lever setting by switching the auto throttle to manual mode and reduced the airspeed to 180 kts. The engine ITT temperature decreased, and the CAS message cleared. He reported that he remained at 10,000ft MSL for several minutes to monitor the engine parameters, and all remained within operating limits. The pilot then contacted ATC to request a small climb to see how the engine would perform, and after receiving authorization, initiated a climb to 13,000ft MSL. Seconds after advancing the engine throttle, there was a loud bang and reduction in power. The pilot reported that he pitched the airplane for best glide, declared an emergency, and received vectors to Athens-Ben Epps Airport (AHN), Athens, GA. While performing the emergency checklist procedure he attempted to “clear the engine,” which aggressively shook the airplane. He continued on vectors, deployed the flaps and landing gear, and made a no power landing on runway 9 at AHN.

A post-incident examination of the airplane revealed an uncontained engine failure and separated section of engine cowl. A section of turbine disk protruded from the lower cowling structure on the right side of the airplane (Figure 1). 

As part of the investigation process, the NTSB invited qualified parties to participate in the investigation, including the Federal Aviation Administration (FAA), Williams International, and Cirrus Aircraft. Party members from the FAA, Williams International, and Cirrus removed the engine at AHN and shipped it to Williams International in Pontiac, Michigan for teardown examination.

Pacific Aerospace 750XL, N221BN, fatal accident occurred on June 14, 2026, at Butler Airport (BUM/KBUM), Butler, Missouri

- History of Flight:
On June 14, 2026, at about 1127 local time, a Pacific Aerospace 750XL, N221BN, was destroyed when it impacted terrain terrain at Butler Airport (BUM/KBUM), Butler, Missouri. The pilot and eleven passengers were fatally injured. The airplane was registered to Skyhi Aero LLC out of Jasper, Tennessee, and being operated by the Skydive Kansas City out of Butler, Missouri, under the provisions of Title 14 Code of Federal Regulations (CFR) Part 91 as a local parachute jump (skydiving) flight. Daylight visual meteorological conditions prevailed at the time of the accident.

According to flight-tracking history, on June 1, 2026, the airplane was ferried out of Skydive Chicago Airport (8N2), Ottawa, Illinois, on a flight to Beloit Airport (44C). The following day, the airplane was flown to Southern Wisconsin Regional Airport (KJVL) and then New Century AirCenter (KIXD), before arriving at KBUM. The airplane conducted two short flights on June 5th, and one short flight on June 6th. Starting from June 12th, the airplane was flown several times on daily bases. The accident flight was the airplane's 13th flight since its arrival at KBUM.

On the day of the accident, at 0920, the airplane departed runway 36 and entered a climbing left turn to an altitude of 13,600 ft before dropping the skydivers over the airport and coming in for a landing on runway 36 (entire flight lasted 22m). At 1032, the airplane departed 36 again and entered a climbing left turn and flew a similar pattern for the earlier flight.

The accident flight was not captured on any flight-tracking or any ADS-B exchange sites.

Several people witnessed the accident, including family members.

Local emergency services reported that the airplane "was not able to make it to full power," and was... "turning left when it stalled near Interstate 49 Business."

The airport manager stated: "It had just taken off and made a left turn [...] I think it was losing power, and he was trying to make it over to the highway and land, and he stalled and went down nose first and caught fire."

The airplane impacted terrain within airport property and a post crash fire ensued.

The accident airplane, serial number 165, was manufactured in 2010. It was an 18-seat (in the parachute variant), low-wing, fixed tricycle gear airplane powered by a Pratt & Whitney Canada PT6A-34 engine that drove a 4-blade MTV-16-1 variable pitch propeller.

According to the aircraft manufacturing company:

According to an old sale listing for the aircraft, it was equipped with the following avionics:

"Garmin GTX 330 ES Transponder Garmin GMA-340 Audio Panel Garmin GPS-695 Garmin GDL-39R data link receiver ARTEX 406 Mhz ELT One blind encoder Aircraft is ADSB out compliant and ADSB in receive only."

- Wreckage and Impact Information:
Given the previous departures and the witness accounts, the airplane was departing runway 36 when it entered a climbing left hand turn, it is unknown weather the reported "engine failure/power loss" happened during the takeoff phase or during the climb/turn.

Given the witness accounts, the airplane's left wing most likely stalled and the airplane entered a nose down attitude before impacting terrain. 

The wreckage was located on the edge of the airport property next to a fence and a treeline separating it from a highway. The entire airplane was located within the main wreckage, and there was no debris field leading to it. Both wings and tail were observed in the debris field. The entire airplane was largely consumed by a post crash fire. The impact appears consistent with a low altitude aerodynamic stall/early spin entry with little to no forward airspeed.

A review of Google Earth imagery showed that there were a number of open fields ahead of runway 36 (see figure 6).

- Airport Information:
KBUM is an uncontrolled public airport located 2 miles north of Butler, Missouri. The airport field elevation is 893.7 ft. The airport features a single runway 18/36 (asphalt/rubberized friction seal coat) which is 3999 x 75 ft.

- Weather:
The nearest weather observation facility, KLRY, located 19 nautical miles north of Butler, recorded the following conditions:

The 1135 METAR included: winds 020 at 8 knots, gusting 14 knots, 10 miles visibility, temperature 24 °C, dewpoint 12 °C, and an altimeter setting of 30.14 inches of mercury. The calculated density altitude was 1980 ft (all these conditions at KLRY).

On June 14, 2026, at 1035 eastern standard time, a Piper PA-28-235 airplane, N15564, was substantially damaged when it was involved in an accident near Key West, Florida. The pilot was not injured and the passenger sustained minor injuries. The airplane was operated as a Title Code of Regulations Part 91 flight.

The pilot stated he rented the airplane and flew from Apopka, Florida to Key West International Airport (EYW), Key West, Florida, the previous day without incident. The next day the airplane was fueled, and he stated the preflight and engine run-up were normal.

He reported that during the accident takeoff, just after rotation, about 60 knots and 60 ft above ground level, the engine lost all power. The pilot said that in an attempt to restore engine power, he confirmed that the engine throttle, mixture control and propeller pitch control were all full forward, but engine power was not regained. At a low altitude and without sufficient runway remaining, the pilot landed straight ahead. The airplane came to rest in a stand of mangrove trees and sustained substantial damage to the wings and fuselage.

The airframe and engine have been retained by the NTSB for further examination.

On May 22, 2026, about 1518 eastern daylight time, a Textron Aviation 172S, N294PL, was substantially damaged when it was involved in an accident near Jupiter Farms, Florida. The flight instructor and student pilot were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.

The flight instructor reported that the airplane departed the North Palm Beach County General Aviation Airport (F45), West Palm Beach, Florida, with a total of 56 gallons of fuel onboard. About 40 minutes into the flight, while at an altitude of about 1,800 ft mean sea level (msl), during a practice climb, the flight instructor observed a decrease in engine rpm from 2,400 to 2,200. The flight instructor assumed control of the airplane; however, the engine rpm continued to decrease followed by a total loss of engine power. Unable to restart the engine, the flight instructor conducted an emergency landing in a plowed field, which resulted in substantial damage to the right wing.

The airplane was recovered and retained for further examination.
