- History of Flight:
On May 2, 2026, at about 0953 local time, a Cosmos Phase II weight-shift-control aircraft, N145W, sustained unknown damage when it was involved in an incident in Prineville, Oregon. The pilot was not injured.
The FAA reported: "Aircraft on landing, pilot reported aircraft "Duckwalked" causing right tire to blow and rolled on its left side."
