- History of Flight:
On September 12, 2026, at about 1246 local time, a Cirrus TRAC20 G6, N486AV, registered to Westwind School of Aeronautics Phoenix LLC and doing business as United Aviation Academy, sustained unknown damage when it was involved in an incident at Ak-Chin Regional Airport (A39), Maricopa, Arizona. The flight instructor and two student pilots were not injured. The training flight originated from Gila Bend Municipal Airport (E63), 
Gila Bend, Arizona, at 1210.

The FAA reported that the airplane was conducting a simulated engine failure to the runway 22 when the engine stalled and landed adjacent to the runway.

- History of Flight:
On September 13, 2026, at about 1855 local time, a Robinson R44 Clipper II, N7527L, registered to Chop Air LLC, sustained substantial damage when it crashed on takeoff in Natchez, Mississippi. The private pilot and three passengers were not injured.

The pilot reported that he landed in an empty to pick up three passengers for a Sunday afternoon ride. "We were just taking off. There were four of us and we were heavy. As soon as I moved to take off, it veered forward and no matter what I did, it wouldn't straighten out."

The pilot, a doctor, held a private pilot certificate (last issued/updated 9/2/2021) with an airplane single engine land and instrument airplane ratings and a helicopter rating. His third class FAA medical was issued on 3/2026 with a note that he must use corrective lens(es) to meet vision standards at all required distances.
