The pilot had completed two previous landings uneventfully and was attempting to land for a third time. Review of airport surveillance video showed that the airplane approached the runway with a significant unarrested rate of descent while in a slightly nose high pitch attitude. The descent continued until the airplane contacted the runway hard, after which the nose landing gear immediately collapsed and the airplane came to stop on the runway. The underside of the airplane’s fuselage was substantially damaged during the landing.

- Probable Cause: The pilot’s improper landing flare, which resulted in a hard landing.

Malta Air, doing business as Ryanair flight 1879, a Boeing 737-8AS, registration 9H-QEU, experienced a No. 2 (right) engine fan-blade-out (FBO) failure during climb out from the Thessaloniki International Airport (SKG), Thessaloniki, Greece. Engine fragments breached the fuselage and the cabin depressurized. The crew elected to return to SKG where they made an uneventful landing. The flight was a scheduled international passenger flight from SKG to Memmingen Airport (FMM), Memmingen, Germany. There were 2 pilots, 4 flight attendants, and 149 passengers (including one lap child) on board. One passenger sustained serious injuries.

The NTSB traveled to the accident site. On July 16, 2026, the Greece Hellenic Air and Railway Safety Investigation Authority (HARSIA) delegated the investigation in full to the NTSB. As part of the investigative process, the NTSB invited qualified parties to participate in the investigation. These included the Federal Aviation Administration (FAA), The Boeing Company, and GE Aerospace. In accordance with the provisions of Annex 13 to the Convention on International Civil Aviation, the countries of Greece (State of Occurrence), Malta (State of the Operator and Registry), and France (State of Manufacture of the Engines, jointly with the United States) designated Accredited Representatives to the investigation, and Malta Air, Safran Aircraft Engines, and the European Union Aviation Safety Agency (EASA) were designated as technical advisors. 

The parties were formed into specialized investigative groups led by NTSB group chairs in the areas of Powerplants, Structures and Survival Factors, and Materials Engineering. The Cockpit Voice Recorder (CVR) and Flight Data Recorder (FDR) were sent to NTSB headquarters for download and specialists were assigned. The materials engineer traveled to France to lead the engine fan blade examination. 

History of Flight 

According to the flight crew, while climbing through an altitude of about 16,000 ft, near Polykastro, Greece, (about 4.5 nm south of the Greek border with Northern Macedonia) they received a No. 2 engine HIGH VIBRATION indication. They reduced the engine power and initiated the Engine High Vibration non-normal checklist. Shortly after, the engine vibration indication was reduced. The crew subsequently continued the climb on autopilot.

The engine vibrations then increased significantly, they heard a loud bang, and the autopilot was disengaged. While assessing the engine condition, the cabin altitude warning sounded. The flight crew donned their oxygen masks, declared an emergency, and began an immediate descent.

The flight attendants (FAs), who were preparing for cabin service, reported hearing and feeling a loud continuous vibration and seeing a small amount of fog/smoke before the oxygen masks dropped. One reported hearing an automated public address system announcement saying “emergency descent”. Another recalled manually making an additional decompression announcement. 

The FAs recalled that passengers had donned their oxygen masks. FA2 and FA4 initially sat down in the forward jumpseats and donned their oxygen masks, however FA4 moved to the galley to use another mask after the one above the jumpseat detached from the overhead. 

FA1 noticed some passengers were standing up and asking for help and responded to assist. The FA noticed that the passenger in seat 11F was partially lodged in a damaged cabin window, and that the entire window was missing. Other passengers were assisting and managed to pull the injured passenger back into the cabin. The FA then sat in seat 11B and donned an oxygen mask. That mask became detached and the FA used a different one. 

A passenger went to the forward galley and asked the FA2 and FA4 for something heavy and metal to try and block the broken cabin window hole and retrieved a metal box to bring back to row 11. 

The injured passenger was subsequently moved to row 12 and was attended to by a passenger who was a doctor throughout the remainder of the flight. The doctor asked for first aid equipment, and FA1 retrieved the forward first aid kit. 

The FAs communicated with the flight deck, confirmed the flight crew were ok, and were informed of the engine failure, decompression, and the intent to return to SKG with an estimated time enroute of 20 minutes. 

The FAs also received instructions to secure the cabin, which they did. The flight crew later informed them when the oxygen masks were no longer necessary. After descending below 10,000 ft, the flight crew secured the No. 2 engine and returned to SKG uneventfully. The airplane remained on the runway for an inspection and then proceeded to the gate, where first responders were waiting.

Crew Experience 

The captain was hired by Malta Air in May 2015. He had accrued a total of 8,270 flight hours, 7,683 of which were in the same make and model as the accident airplane. The first officer was hired by Malta Air in March 2023. He had accrued a total of 2,523 flight hours, 2,333 of which were in the same make and model as the accident airplane.

Airplane Examination 

Examination of the airplane revealed that the No. 2 engine, an CFM International (a joint company between Safran Aircraft Engines and GE Aerospace) model CFM56-7B26, experienced a fan blade separation that resulted in engine fragments striking the fuselage and the right horizontal stabilizer. The pressurized section of the fuselage was penetrated 1) through the passenger cabin window located at row 11, as well as 2) lower lobe fuselage skin behind the right wing-to-body fairing, see figures 2 and 3. Additional impact damage was found on the right wing-to-body fairing, a dent with paint smearing on the fuselage window belt skin, a dent on a stringer common to the dented fuselage window belt skin, a gouge on the right horizontal stabilizer upper skin and a dent on the right horizontal stabilizer leading edge. 

Examination of the engine and nacelle revealed that one fan blade (No.10) had fractured and separated below the blade platform. The outer blade tips were fractured and separated on two additional fan blades (Nos. 16 and 21). All remaining blades were full length but exhibited varying levels of impact deformation. Three loose fan blade fragments were recovered from the engine. A visual examination of the core was performed and there was no evidence of radial uncontainment though the engine cases, or any undercowl fire. The engine inlet cowl, which is forward of the fan case, exhibited evidence of punctures as described below.

The inlet and fan cowling remained attached to the engine. The inlet cowl inner barrel exhibited impact damage, tearing, and/or missing material from the 3 to 9 o’clock (as viewed from the rear) positions, see figure 4. The inlet lip skin had a scrape mark at approximately the 7 o’clock position. 

The inlet cowl outer barrel had a radial penetration, forward of the fan plane of rotation at the 9:30 position that measured approximately 14 inches circumferentially by 10 inches axially. A second inlet cowl tear and puncture was present forward of the fan plane of rotation at the 7 o’clock position that measured approximately 13 inches circumferentially by 10 inches axially, see figure 5. 

Several of the aft bulkhead splice fasteners failed at the 9:00, 6:00, and 3:00 o’clock positions. The attach ring to aft bulkhead fasteners failed 360 degrees circumferentially and loose rivet tails were observed along the bottom of the inner barrel. Enhancements for fasteners, defined in Federal Aviation Administration airworthiness directive (AD) 2025-04-01, had not been incorporated on this inlet cowl hardware. 

The inboard fan cowl sustained skin damage near the integrated drive generator (IDG) oil access door, and the No. 1 forward fan cowl latch was found disengaged after the event. The gap between the inboard fan cowl and thrust reverser was reduced at the bottom of the interface with contact observed at the forward edge of the thrust reverser. Enhancements for fasteners and an external doubler, defined in AD 2025-04-02, had not been incorporated on this fan cowl hardware.

Bird remains, including feathers, were recovered from the forward face of IDG oil cooler, one of the thrust reverser blocker door drag links, and the lower (6 o’clock position) of the engine fan case near the thrust reverser split flange. The remains, along with the samples from the fan blades described below, were hand carried to the Smithsonian Institution Feather Identification Lab in Washington, DC, for examination. 

The exhaust nozzle remained attached to the engine. There was cracking and 360° shear buckling on the exhaust case immediately aft of the engine turbine rear frame. Enhancements for installing additional brackets for the exhaust nozzle, defined in AD 2025-04-03, had not been incorporated on this exhaust nozzle hardware.

The FAA compliance date for all three of these ADs is July 2028. 

Fan Blades Examination 

Pieces from the fractured No.10 fan blade, the remaining 23 fan blades, and associated shims, spacers, and platforms were sent to Safran Aircraft Engines in Moissy-Cramayel, France for a group examination led by the Chief Technical Advisor for the NTSB Materials Laboratory, see figure 6 for relative location of shims, spacers and platforms to blades.

The No. 10 fan blade was fractured through the dovetail in the blade root as shown in figure 7. A portion of the fracture surface covering approximately 37 percent of the total fracture surface had relatively smooth features with a curving boundary, features consistent with fatigue. The fracture features were traced back to an origin area on the concave side of the blade near the trailing edge.

At the fatigue origin area located about 0.559 inch (14.2 mm) radially away from the root end face, ratchet marks were observed, consistent with multiple origins along an axial length of about 0.650 inch (16.5 mm). Heavy fretting damage was observed locally on the dovetail contact face adjacent to the origin area and in the same area on both sides of the associated shim.

The fracture surface on the No. 10 fan blade dovetail piece was examined using a scanning electron microscope. The origin areas were damaged from fracture surface recontact and rubbing. However, fatigue striations were observed across the fatigue region from 0.008 inch (0.2 mm) deep to the boundary at a depth of 0.626 inch (15.9 mm). 

Striation spacing was relatively coarse and unchanged across the fatigue region, consistent with fracture due to high amplitude fatigue (HAF). In aircraft engine fan blade fractures, HAF is associated with cyclic loading that occurs at frequencies greater than a flight cycle and at stress amplitudes that are larger than those typically associated with high-cycle fatigue (HCF). 

The No. 10 spacer exhibited an area of elastomer deterioration near the trailing edge end, see figure 8. 

Visual examinations were completed on the remaining 23 fan blades. Fan blade Nos. 8, 9, 12, and 21 had rounded deformation of the leading edges. The blades were further examined for evidence of organic material transfer using black light. Samples of fluorescing material were collected from blade Nos. 9, 11, 12, 20, 21, and an airfoil fragment from blade 10. 

Recorders 

An initial review of quick access recorder data revealed several evolutions of vibrations were detected by two sensors (one located at bearing No. 1, the other on the fan frame compressor case (FFCC)) for the No. 2 engine during take-off and climb. Vibration values for the FFCC sensor were higher than the bearing No.1 sensor during these phases and are provided below.  

The first was a sudden increase simultaneously from both sensors near the end of the take-off roll. The FFCC value increased from about 0.2 cockpit units (CU) to 1.7 CU before the airplane lifted off. The vibration value then rose from 1.7 to about 2.0 CU over the next 175 seconds. The rate of vibration change then increased, and the values rose from 2.0 CU to 4.3 CU over the next 250 seconds, until the engine power was reduced. As the power reduced, the vibration value decreased to about 1.2 CU. As the power was subsequently increased, the level rose first from 1.2 CU to 2.2 CU, then from 2.2 CU to the maximum recording limit of 5 CU within 19 seconds.

The Honeywell solid state FDR readout is in progress at the NTSB Vehicle Recorder Laboratory. 

The airplane was equipped with a Honeywell solid state CVR that recorded the last two hours of aircraft operation across four channels. The event flight was about 57 minutes in duration and was captured in its entirety. Both English and Greek were spoken by the flight crew during the flight. Some cockpit discussions were not discernable because they were obscured by simultaneous radio transmissions.

About seven and a half minutes after the start of the takeoff roll, the crew called for the high engine vibration non-normal checklist, noting that the vibration level was 4.3. No aural alert was audible on the CVR. A loud bang sound was recorded about nine minutes and twenty seconds after the start of the takeoff roll.

The No.2 electronic engine control (EEC) unit and the airborne vibration monitor (AVM) unit were retained for data downloads at their respective manufacturers at a later date.

Maintenance Records 

A review of maintenance records for the engine revealed that the fan blades had undergone ultrasonic inspections in accordance with CFM service bulletin (SB) 72-1033 revision 3, on November 11, 2025, and most recently on May 24, 2026, 253 cycles before the accident flight with no findings. It prescribes initial and recurring ultrasonic inspections of the concave and convex side of the fan blade dovetail, for any indications of crack development. The service bulletin was originally issued in 2018 following an FBO event in which a passenger was fatally injured.  

Four suspected bird strikes to the accident airplane’s No. 2 engine were reported by flight crews in the 12 months preceding the accident. No damage was found in the subsequent maintenance actions. Bird remains were reportedly found in two of the cases.

Cabin Examination 

Examination of the airplane cabin revealed that the only emergency equipment found opened and used was a first aid kit found on seat 12F. All oxygen masks were found to be deployed.

The forward flight attendant seat restraints were found all buckled, and aft flight attendant seat restraints were found unbuckled. All galley bins located in the forward and aft R1 were found stowed. Some of the locks and latches on the galley carts and bins were found in the unlatched position. The metal box mentioned in the flight attendant statement was found in the R1 aft galley, inboard second to the top compartment with no notable damages.

Middle and outer passenger windowpane shards were found in the cabin area longitudinally between passenger rows 8 and 15. The interior sidewall panel surrounding the damaged passenger window at row 11, had a fracture at the lower aft vertical edge of the window that extended aft, to the nearest fuselage frame aft of the window edge.

Additional Investigation 

Notes The No. 2 engine, along with its inlet, the No. 1 engine fan blades, shims and spacers, and the row 11 passenger window forging were retained for the investigation. 

The investigative team is aware of previous FBO events with similar engine models that resulted in damage to engine inlets or cowlings and fuselage structures. Determination of any relevant similarities or details between this accident and previous events remains under investigation. 

This investigation is ongoing.

On August 1, 2026, about 2301 mountain daylight time (MDT), a Cessna 172G, N1166F, was substantially damaged when it was involved in an accident near Blackfoot, Idaho. The pilot was fatally injured, and the two passengers sustained serious injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

A witness reported the pilot completed 3 takeoffs and landings for night currency before departing on the accident flight about 2300 MDT. 

Law enforcement received a 911 call reporting an airplane crash on a road adjacent to the airport, about 2301 MDT. No witnesses, surveillance video, or flight data captured the accident flight.

The airplane's first point of contact (FPOC) was marked by a ground scar about 33 ft inside the airport's eastern perimeter fence near the northeast corner of the airport. Paint chips, the rightwing tip and other debris extended from the FPOC through the perimeter fence to the main wreckage, which came to rest upright on the adjacent road about 30 ft beyond the airport boundary fence. The debris path was oriented about 058° magnetic, and the airplane came to rest on a heading of about 189° magnetic. 

Crushing and deformation were observed throughout the fuselage, wings, empennage, cockpit, and landing gear. The right wing exhibited leading-edge crushing and compression, with buckling of the upper and lower wing skins. The wing was bent upward near the root, with deformation extending into the midspan section of the wing. Portions of the wing skin were torn and separated. The left wing exhibited extensive buckling of the outboard wing skin and leading edge. The outboard wing was bent upward and aft, with wrinkling extending inboard.

The fuselage was crushed, with substantial damage to the cabin and aft fuselage. The vertical stabilizer and rudder exhibited crushing and deformation. The horizontal stabilizer and elevator were bent and partially separated at their attachment points. 

The two-bladed McCauley propeller was separated from the engine crankshaft and was located within the debris field. Both blades exhibited extensive chordwise scratches along their surfaces. One blade was separated at the blade tip. The other blade exhibited bending near the outboard end, with the blade tip bent aft along with deformation. The propeller spinner was crushed and extensively deformed.

The airplane was retained for further examination.

On July 14, 2026, about 1725 Pacific daylight time, a Piper PA-24 airplane, N5002P, was substantially damaged when it was involved in an accident at Rogue Valley International/Medford Airport (MFR), Medford. Oregon. The two pilots were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. 

The flight instructor reported that he and the pilot had intended to familiarize themselves with the airplane after it had been purchased by a new owner. Prior to the accident flight, the new airplane owner reported to the flight instructor that the prebuy inspection had identified some landing gear anomalies. Most of the anomalies were addressed during subsequent maintenance with exception of a delayed landing gear annunciation, which they were unable to duplicate. During the accident flight, one of the pilots asked the air traffic controller to observe if the landing gear was down. On the third circuit in the traffic pattern, the pilots heard the landing gear extend; however, the gear indication for the down and locked position did not illuminate. The pilot asked the tower controller to observe the landing gear position again, and the controller confirmed it was down. Nevertheless, about 200 ft - 300 ft above ground level, the landing gear down and lock position light illuminated and the pilot continued the approach and landing. 

Shortly after touchdown, during the landing roll, the landing gear collapsed and the airplane subsequently came to rest on the runway. Postaccident examination by a FAA inspector revealed that the engine mount sustained substantial damage. 

The airplane was recovered and retained for further examination.

Cessna 208B Grand Caravan EX, N247JP, accident occurred on August 9, 2026, near Lemhi County Airport (SMN/KSMN), Salmon, Idaho

On August 9, 2026, at about 1046 local time, a Piper PA-32-300 Cherokee Six, N3011X, registered to Hanania-Stout Motorcars LLC, sustained substantial damage when it was involved in an accident near Dawsonville, Georgia. One occupant suffered serious injuries, another suffered unknown injuries. The flight originated from Chattanooga Metropolitan Airport/Lovell Field Airport (CHA/KCHA), Chattanooga, Tennessee, at 1114.

Preliminary ADS-B data shows that the airplane departed CHA and entered a climb to an altitude of 7,200 ft. About 23 minutes into the flight, the airplane initiated a descent towards Elliott Field and entered a late short final for the runway and ended up impacting heavily wooded terrain east of the airport. There was no post crash fire.


The FAA reported that the airplane declared an emergency after reporting engine issues and crashed into trees. The NTSB is investigating.

Elliott Field is a private use airport located 4 miles northwest of Dawsonville, Georgia. The airport field elevation is 1497 ft. The airport features a single runway 14/32 which is 5619 x 40 ft.

The airplane departed TA17 at 2125 LT and headed north before returning back towards the departure area.

On July 12, 2026, about 1705 mountain daylight time, a Kaman Aerospace Corporation K-1200 (K-MAX) helicopter, N40HX, was substantially damaged when it was involved in an accident at Silver Jack Reservoir, Colorado. The pilot was fatally injured. The helicopter was operated as a Title 14 Code of Federal Regulations (CFR) Part 133 external load firefighting flight.

The helicopter was operated under a call-when-needed contract with the United States Forest Service (USFS) and was conducting water bucket/fire suppression flight for the Gold Mountain Fire in the Uncompahgre Wilderness of the north-central region of the San Juan Mountains in Colorado. The firefighting helicopter was equipped with a 660-gallon Bambi MAX Bucket with PowerFill Max (water bucket) suspended from the helicopter by a 150 ft long line.

The helicopter was equipped with an Appareo AIRS-400 airborne image recording system that recorded video, ambient audio, and flight parameter data during the flight. The recovered device was examined at the NTSB Vehicle Recorder Laboratory, Washington, DC.

According to recovered flight parameter data, at 1607:41, the helicopter lifted into a hover at a remote helibase near Ouray, Colorado. At 1608:17, the helicopter’s ground speed and altitude began to increase, consistent with the helicopter departing the helibase.

The helicopter initially flew north-northwest along Hwy 550 toward Ridgway, Colorado, before it turned northeast and flew to the north end of Silver Jack Reservoir. The helicopter made a descending right 270° turn on the east side of reservoir. The helicopter then flew to the northnortheast over the reservoir where it refilled its water bucket. The helicopter then conducted firefighting operations about 2.7 nautical miles (nm) south-southeast of the reservoir. Based on the available data, the helicopter completed 5 water bucket drops over the forest fire.

Review of the recovered video content confirmed that visual meteorological conditions prevailed throughout the flight and the water bucket dip locations at the reservoir included a visible horizon, shoreline, and surrounding terrain features for vertical reference during external load operations. 

Review of the previous five water bucket dips revealed that the pilot generally made large repetitive cyclic inputs while he maintained hover and were comparable to the large cyclic inputs that were observed during the final bucket dip that immediately preceded the accident. The enroute portions following the previous five water bucket dips, each with an external load of about 3,000 lbs, were unremarkable.

Review of the previous five water bucket drops revealed the pilot generally made large repetitive cyclic inputs while he maintained hover before the water drop. Immediately after each water release and after the helicopter transitioned to forward flight, the pilot’s flight control inputs were unremarkable while the helicopter was enroute to the reservoir dip sites.

During the final water bucket dip, the helicopter pitched up rapidly, accelerated in right yaw, and rolled right into an extreme unusual attitude. The turn/slip indicator showed full right deflections. The external load indicator rapidly increased from about 4,000 to about 7,700 lbs. A large aft cyclic movement was observed as the unusual attitude progressed and the cyclic continued moving aft until it was obscured in the field of view (FOV). The helicopter pitch then decreased rapidly, and the trees, shoreline, and water came back into the FOV. Based on external visual references and the attitude indicator, at the end of the video the helicopter was in an extreme right roll with the nose falling through the horizon toward the water.

The portions of the video that were reviewed, which included the final sequence of events, did not reveal any abnormal sounds or vibrations indicative of a rotorcraft system or engine malfunction during the flight.

There were two witnesses positioned on the dam at the north end of the reservoir. They reported seeing the helicopter enter the reservoir from the south to refill the water bucket. One of witnesses reported that after the helicopter refilled its water bucket it turned right (east) and descended into the reservoir upside down.

On July 22, 2026, the helicopter, water bucket, and 150 ft long line were recovered from the reservoir using a Boeing CH-47 Chinook helicopter. The wreckage was then loaded on a trailer and transported to a secure storage facility where it was examined.

Examination of the helicopter revealed impact-related damage primarily to the right-side of the fuselage. There were only fragments remaining of the windshield and door windows. The cockpit floor was displaced upward due to the impact-related damage to the right fuselage. The left side of the fuselage exhibited minor damage when compared to the right side of the fuselage.

The cyclic control system exhibited mechanical continuity from the cyclic stick to the L-cranks at rotor hubs, and then from the L-cranks to the spanwise blade control rod. Movements in forward, aft, left and right had friction from impact-related fuselage damage throughout the flight control system.

The collective control system exhibited continuity from collective stick to the rotor hub Lcranks, and then from the L-cranks to each spanwise blade control rod. Collective movements in up and down had friction from the collective stick friction block being dislodged and rubbing against the airframe.

The directional pedals had full range of movement in the cockpit with mechanical continuity from the pedals to the rotor hub L-cranks, and then to each spanwise blade control rod. The rudder control cable between the directional pedals and the rudder exhibited continuity from the pedals to aft section of the fuselage where the cable was cut to facilitate disassembly for transportation. The left side of the fuselage below the cockpit exhibited impact-related damage causing some binding of the rudder control rod below the cabin floor. The portion of the rudder control cable remaining in the aft tail section exhibited free movement and smooth control of the rudder surface.

Examination of the control module appeared to have full range of movement allowing the cyclic, collective and directional pedals all to have full continuity to their respective systems. Impact-related damage to the control module mounts displaced the module up from its normal position.

Based on photos of the helicopter at the accident site, the right stabilizer was observed partially separated from the aft fuselage. Examination revealed impact-related damage to the right side of the aft fuselage that pushed the left- and right- stabilizer bearings out of their supports. Additionally, the stabilizer system exhibited binding due to impact-related damage at the stabilizers. To relieve the binding, the control rod was disconnected from the stabilizer tube, which allowed full range of movement throughout the entire stabilizer system. 

Examination of the N1 twist-grip engine control confirmed full continuity and freedom of movement to the engine connections. The N2 RPM engine control exhibited mechanical continuity and freedom of movement to the engine connections. Collective movements up and down were confirmed but had friction from the collective stick friction block that was dislodged and rubbing against the airframe.

Examination of the transmission, mounts, surrounding structure, and rotors shafts revealed no evidence of damage. The rotor hubs rotated freely by hand and/or through the input drive shaft. The transmission rotated freely, without any noticeable anomalies, in both directions. The freewheeling unit remained attached to the transmission input and operated as designed. Counterclockwise rotation of the clutch verified freedom of movement and no sprag engagement.

To confirm the transmission oil pump was still operational, the NR tachometer was removed from its respective mount pad on the transmission oil pump. The transmission was rotated by hand through one of the rotor masts, and the tach drive rotated which confirmed the transmission oil pump was not seized.

Examination confirmed that the left- and right-rotor hubs remained intact and exhibited minor impact-related damage. The droop stops exhibited damage and were not functional. The rotor hubs exhibited evidence of external damage from contact with blade grip stop assemblies, or where stop assemblies were missing.

Examination of right rotor blade, s/n 0516B, was initially found impinged under the left U-Crank, s/n 0513B, with the trim actuator bracket broken off. There was no leading-edge damage observed to the rotor blade where it contacted the lower side of the left rotor hub. Both left and right rotor blade sets remained installed to their respective rotor hubs but were each severed inside of blade station 74. All lag pins exhibited intact safety devices and there was torque present when removed. 

Examination and layout of the left rotor blade, s/n 0513A, revealed about 50% of the blade was recovered with most of the pieces belonging to inboard of blade station 189. Additional recovered pieces began outboard of blade station 246 to the tip. Spanwise control rod was found intact but ripped out of the blade. The control rod remained connected to the blade stub and to the flap bell crank, control rod, flap control horn bearing, and 3 portions of blade afterbody. The servo flap and support brackets were not located. 

Examination and layout of the left rotor blade, s/n 0513B, revealed about 65% of the blade material was recovered, with identified portions comprising the entire rotor span. The spanwise control rod was broken inside the inboard blade stub, and the remaining control rod was not located. The flap bell crank and associated hardware were not located in the blade pocket. The inboard and outboard flap brackets with flap support rod were present. The servo flap was not located. A 4.5-foot section of stainless-steel blade leading-edge was found on the reservoir shoreline and exhibited multiple bends, wrinkles, and scrapes. Additionally, a few pieces of blade weights were in the same area.

Examination and layout of right rotor blade, s/n 0516A, revealed about 35% of blade material was recovered with mainly larger identifiable pieces inboard blade station 108. Additionally, a 3 ft section of the diagonal strap portion of blade with inboard flap bracket and flap rod still connected were located. The outboard flap bearing was found separated from the rod. The spanwise control rod remained attached to blade stub that was still attached to rotor hub. The servo flap was not located. The inboard flap bracket and flap support rod was located. The outboard flap bracket was not located.

Examination and layout of the right rotor blade, s/n 0516B, revealed about 35% of blade material recovered with mainly larger identifiable pieces inboard blade station 108. The spanwise control rod remained attached to blade stub that was still attached to rotor hub. The servo flap and support brackets were not located.

Examination of the recovered debris identified one right leading-edge guard along with a few upper and lower skin fragments. There were no additional remnants of servo flaps recovered. The severity of damage and lack of recovered servo flap materials prevented the identification of any specific servo flap assembly.

Examination of the engine case and accessories found no notable damage or anomalies. Turning the K-Flex driveshaft showed continuity of the N2 system, which rotated with no notable binding. About 2-3 oz of fuel was drained from the main supply line to flow divider. The fuel was straw colored and free of water or particulate contamination. There was oil present in all oil supply and return lines. Mechanical control continuity to the fuel control unit and power turbine governor was confirmed through movement of the cockpit controls.

The left- and right-hand air particle separators were removed. Inlet guide vanes and first stage compressor blades from the engine were found in the particle separators. 

Visual examination looking into the inlet found extensive damage to the variable inlet guide vanes and all stages of compressor blades. The compressor topcase was removed to facilitate further examinations of the compressor section.

All 1st, 2nd, 3rd, and 5th stage compressor blades were fractured at the blade roots consistent with overload failure. One 4th stage blade was fractured at the blade root. All other 4th stage blades showed foreign object debris (FOD) damage and were bent in direction of rotation. The centrifugal impeller showed extensive FOD damage.

The combustor turbine assembly was removed to facilitate visual examination of the gas producer and power turbine areas. All areas in the gas producer and power turbine were found to be unremarkable and exhibited normal operational wear. Some compressor debris was found in the deflector and 1st stage gas producer nozzle.

Based on initial photos of the helicopter at the accident site, the 150 ft long line remained attached to the airframe cargo hook installed on the helicopter’s lower fuselage. The long line remained attached to the water bucket wire rigging. The water bucket remained attached to the wire rigging and exhibited no damage when examined after recovery. The water bucket valve was found in the closed position. 

The airframe cargo hook mechanical release mechanism functioned normally when tested by hand. The mechanical release cable functioned normally when tested using the release control in the cockpit. The mechanical release cable moved smoothly without any binding or roughness.

The helicopter wreckage will remain in secure storage pending additional examination and testing.
