- History of Flight:
On July 30, 2026, at about 2354 local time, a Cessna 172M Skyhawk, N9650V, registered to a private individual and being operated by Blitz Aviation out of Ogden, UT, sustained substantial damage following a forced landing near Hill Air Force Base (HIF/KHIF), Ogden, Utah. The pilot and passenger suffered minor injuries. The flight originated from Nephi Municipal Airport (U14), 
Nephi, Utah, at 2300, and was destined to Ogden Municipal Airport (OGD/KOGD), Ogden, Utah.

The FAA reported that the airplane experienced engine failure and made a forced landing on a road short of the runway and flipped over.

Preliminary ADS-B data shows that the airplane was cruising at ~7,000 ft when it entered a descent towards KHIF but ended up landing short on a road.

On June 20, 2026, about 1134 eastern daylight time (EDT), Delta Air Lines flight 2351 (DAL2351), an Airbus A319, and American Airlines flight 3161 (AAL3161), a Boeing 737-800, were involved in a runway incursion that resulted in a go-around of DAL2351 at General Edward Lawrence Logan International Airport (BOS), Boston, Massachusetts. There were no injuries reported to the 131 passengers and 5 crewmembers on board DAL2351 or the 176 passengers and 6 crewmembers on board AAL3161. There was no damage to either airplane.

DAL2351 was a scheduled domestic passenger flight operating under the provisions of Title 14 Code of Federal Regulations (CFR) Part 121 from Dallas Fort Worth International Airport (DFW), Dallas-Fort Worth, Texas, to BOS. AAL3161 was a scheduled domestic passenger flight operating under the provisions of Title 14 CFR Part 121 from BOS to Charlotte-Douglas International Airport (CLT), Charlotte, North Carolina. At the time of the runway incursion, DAL2351 was descending to land on runway 33L when AAL3161 began its takeoff roll on runway 27. After the go-around, DAL2351 landed in BOS without incident while AAL3161 continued their takeoff and completed their flight to CLT without incident.

As part of the investigative process, the NTSB invited qualified parties to participate in the investigation. Parties to the investigation include the Federal Aviation Administration (FAA), National Air Traffic Controllers Association (NATCA), Delta Air Lines, American Airlines, Air Line Pilots Association (ALPA), Allied Pilots Association (APA), and Boeing. In accordance with the International Civil Aviation Organization (ICAO) Annex 13, the Bureau d’Enquêtes et d’Analyses pour la Sécurité de l’Aviation Civile (BEA) of France, representing the State of Design of the Airbus A319 airplane, was notified of the investigation. The parties were formed into specialized investigative groups led by NTSB group chairmen in the areas of Air Traffic Control (ATC), Operations, and Human Performance. Additionally, NTSB specialists in the areas of Meteorology, Aircraft Performance, and Recorders were assigned to the investigation.  

At the time of the incident, a local control west (LCW) controller, a local control east controller (LCE), a ground control (GC) controller, a clearance delivery (CD) controller, and an operations supervisor (OS) were present in the BOS ATC tower cab. The LCW controller was responsible for aircraft departure and arrival operations on runways 27 and 33L. The LCE was responsible for traffic coordination, monitoring the LCW position, writing flight strips, and local helicopter traffic. See figure 1 below for a BOS airfield diagram.

Certified ATC audio recordings and Automatic Dependent Surveillance-Broadcast (ADS-B) data for the incident were provided by the FAA. Table 1 provides key transmissions between the LCW controller and the flight crews of DAL2351 and AAL3161 as well as key events from ADSB data.

A preliminary estimate of the closest proximity between DAL2351 and AAL3161 determined they were about 670 ft apart, as shown in figure 2. 

Airport Surface Detection Equipment – Model X (ASDE-X) 

BOS was equipped with Airport Surface Detection Equipment – Model X (ASDE-X), a system that provides ATC controllers with visual and aural alerts to runway conflicts through predictive system logic. BOS was also equipped with Runway Status Lights (RWSL), an automated system that provides situation awareness to flight crews and vehicle operators regarding potential conflicts at runway and taxiway intersections that are detected using surface radar surveillance. The RWSL system at BOS included takeoff hold lights (THL) on runways 9/27 and 15R/33L. The THL illuminate red when the system control logic determines it is unsafe to depart the runway due to a predicted conflict and extinguish when there is no predicted conflict.

A preliminary review of the ASDE-X data from the incident by the FAA determined that the system generated an aural and visual alert for a predicted conflict involving converging targets, specifically DAL2351 and AAL3161. Additionally, a preliminary analysis by the FAA of the performance of the RWSL determined that the THL for runway 27 had illuminated while AAL3161 was lining up at the threshold of runway 27 and GPD82 was approaching runway 33L to land. After GPD82 had landed and was about to cross the intersection with runway 27, the THL for runway 27 extinguished.

The THL for runway 27 illuminated red again when DAL2351 was on final approach to runway 33L, and subsequently extinguished when DAL2351 was crossing the intersection with runway 27. 

Both DAL2351 and AAL3161 were each equipped with a cockpit voice recorder (CVR) that recorded a minimum of 2 hours of cockpit audio. By the time investigators requested the CVRs be quarantined for each airplane, the CVR data for the incident flight had been overwritten. The digital flight data recorder (DFDR) from each airplane still contained flight data from the incident. These data are currently being analyzed by investigators.

Delta Air Lines Flight 2351 (DAL2351) Flight Crew Statement

Postincident statements provided by the DAL2351 flight crew revealed the first officer (FO) was the pilot flying and the captain was the pilot monitoring. The captain stated that, once Tradewind Aviation flight 82 (GPD82), a Pilatus PC-12, passed through the intersection of runways 33L and 27, they heard AAL3161 receive a takeoff clearance and their understanding was that AAL3161 would depart and be clear of runway 33L before their arrival. The captain recalled seeing AAL3161 move forward slightly, and then stopped and remained stationary. The captain told the FO he would monitor GPD82 until it cleared runway 33L as well as the progress of AAL3161 through the runway intersection.

The captain recalled seeing AAL3161 initiate its takeoff roll at about 500 ft, and as AAL3161 approached the 1,000-ft markers on runway 27, the captain instructed the FO to initiate a goaround, which the FO stated he immediately executed. The FO stated he was unable to see AAL3161 during initiation of the go-around due to obstruction from the cockpit window pillar that was within his field of view. The captain stated they did not receive a traffic alert and collision avoidance system (TCAS) alert at any time during the incident.

American Airlines Flight 3161 (AAL3161) Flight Crew Interviews 

Postincident interview of the AAL3161 flight crew revealed that the taxi to runway 27 was uneventful. The FO was the pilot flying and the captain was the pilot monitoring. When AAL3161 was cleared to take off, the FO advanced the engine thrust levers to 40% power. After the engines stabilized, the FO pressed the takeoff/go-around (TOGA) button and then received a takeoff configuration warning; the FO immediately pulled engine power back to idle. The captain checked the flaps, trim, parking brake, and speedbrake to verify that the takeoff configuration was correct. 

After the flight crew concluded the takeoff configuration warning was likely erroneous, the FO advanced the thrust levers for take off. When approaching the intersection of runways 27 and 33L, the captain stated he saw DAL2351 initiating a go-around. The AAL3161 flight crew determined they would not be in further conflict with DAL2351 and elected to continue their takeoff. Both the captain and FO stated they did not see any RWSLs illuminated. 

Boston Air Traffic Control Tower Interviews

Postincident interviews of the BOS ATC tower LCW, LCE, and OS revealed that there were no notable distractions or anomalies within the tower cab during the time leading up to the runway incursion. The controllers interviewed characterized both the traffic and complexity as moderate-to-low at the time of the incident. The LCW recalled that, after seeing GPD82 land, he cleared AAL3161 for takeoff and thought he saw the airplane begin its takeoff roll. Believing that AAL3161 would continue its takeoff, and not receiving further communication from the crew of AAL3161, the LCW stated he subsequently focused his attention back on GPD82 to ensure they would vacate runway 33L in time for DAL2351 to land.

The LCW recalled hearing the ASDE-X alert, looked out the tower cab window, and recalled feeling confused since GPD82 was exiting runway 33L at that time. The LCW stated he normally canted the flight strip on the strip bay after giving a takeoff clearance, but saw the flight strip for AAL3161 was not canted when the runway incursion occurred, leading him to believe that he must not have cleared them for takeoff. The LCE recalled that, during the time leading up to the runway incursion, he was writing flight strips for the LCW and was also monitoring GPD82 as the LCW was providing them multiple taxi instructions. 

The LCE stated he became aware of the runway incursion when DAL2351 began to communicate they were going around, after which the ASDE-X alerted, and he saw DAL2351 performing a go-around while AAL3161 was rolling down runway 27. The OS stated that, prior to the incident, he was contacted by a supervisor at the Boston Consolidated Terminal Radar Approach Control (TRACON) about a prior pilot deviation. The OS was looking through their logs to research this prior pilot deviation when he heard the LCW instruct GPD82 to expedite their taxi on runway 33L. The OS recalled looking at the intersection of runway 33L and taxiway T when he saw the nose of DAL2351 rise up, after which the ASDE-X alerted. 

The NTSB’s incident investigation is ongoing.

On July 13, 2026, about 800 mountain daylight time, a Tag Aviation Pty Ltd Titanium Explorer gyrocopter, N665TE was substantially damaged when it was involved in an accident near Larkspur, Colorado. The pilot passed away in the hospital five days after the accident. The gyrocopter was operated under Title 14 Code of Federal Regulations Part 91 as a personal flight. 

According to a friend of the pilot, the pilot flew the gyrocopter from Meadowlake Airport (FLY) Peyton, Colorado, to Central Colorado Airport (AEJ) Buena Vista, Colorado, 4 days before the accident. The pilot and his friend flew their respective gyrocopters around the Buena Vista area several times during that four-day period. The friend said that the gyrocopter was performing normally. 

On the morning of July 13th, the pilot filled the fuel tank with 91 Octane motor gas before he departed back toward FLY. The friend said that the pilot planned to circumnavigate Davis Airfield (AFF) Colorado Springs, Colorado, to the north and then fly back toward the east toward Peyton, Colorado. The pilot departed AEJ about 700. While enroute, he called another friend on his cellular phone to let him know that he was flying over Lake George in Park County, Colorado. He told the friend that everything was fine and he would call him again when he landed safely at FLY. 

About 800, a witness walking her dog outside her home said that she heard the gyrocopter fly overhead. She said the gyrocopter was low and the engine was “struggling.” Then, the engine stopped and the gyrocopter fell through the trees. She reported that the pilot was conscious but not speaking and that fuel was dripping out of the fuel tank. 

The gyrocopter was relatively intact and came to rest on its right side under a group of trees. The rotor blades were bent and cracked. One of the rotor blade tips was sheared off. The three-bladed propeller was still attached at the hub, and the blades were flat and smooth. The needle on the Aviasport gyro rotor tachometer was fully in the red zone at 500 rpm. 

According to the engine and airframe logbooks, N665TE would have been due for an annual condition inspection in September 2026. At the time of the accident, the airframe had accumulated about 50 hours, and the engine had accumulated about 400 hours. 

The gyrocopter was retained for further examination.

On July 10, 2026, about 1103 Pacific daylight time, a Canadair CF-5D, N845PS, was substantially damaged when it was involved in an accident near Mojave, California. The two pilots onboard were not injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight. 

The pilot reported that during the landing roll the left engine fire warning system initiated, which was followed by a right engine fire warning. He stated that he performed the emergency procedure and the airplane came to a stop on the runway, at which time both pilots egressed. 

A witness photographed the airplane in the airport’s traffic pattern prior to landing. A photograph showed the airplane trailing white vapor from the left side of the fuselage underneath the left wing and around the left engine exhaust. 

An airport surveillance video showed the airplane touchdown on the runway with white vapor trailing. The airplane continued to roll down the runway when the white vapor stopped and the airplane caught fire. The airplane came to a stop on the runway where the fire continued to burn near the empennage. 

The airplane sustained substantial damage to the fuselage and empennage.

The airplane was relocated to a hangar for further examination. 

Piper PA-28-180 Cherokee C, N9062J, fatal accident occurred on July 28, 2026, near Chandler Municipal Airport (CHD/KCHD), Chandler, Arizona

On July 28, 2026, at about 1949 local time, a Piper PA-28-180 Cherokee C, N9062J, registered to True to Life Productions Inc out of Snowflake, Arizona, was destroyed when it impacted terrain shortly after takeoff from Chandler Municipal Airport (CHD/KCHD), Chandler, Arizona. The pilot was fatally injured.

The airplane was recently sold by the registered owner mentioned above. Flight-tracking history shows that the airplane departed on two local flights out of CHD on July the 24th and 27th.

According to Automatic dependent surveillance-broadcast (ADS-B) data, at 1948:25, the airplane was first observed on takeoff from runway 22R. At 1948:46, the airplane was seen approaching the departure end of the runway at 71 knots groundspeed. At 1948:55, the airplane crossed the departure end and was at 1,500 ft and 66 knots groundspeed. For the next few seconds, the airplane continued to climb, but the groundspeed kept going down. At 1949:25, the airplane reached the crest of its climb at 2,000 ft and 61 knots groundspeed when it stopped climbing and entered a shallow descent. At 1949:38, the last ADS-B data point was recorded about 0.80 mile from the departure end at ground elevation with a reported groundspeed of 46 knots and -320 feet per minute vertical rate.

The actual accident site was located located ~0.81 mile from the departure end of runway 22R at the edge of a commercial parking lot. The accident site was also located about 600 ft and to the right side from the last recorded ADS-B data point.

Dashcam video footage showed the airplane in an aggressive right bank/roll and appeared to be slowly recovering before it impacted terrain out of view and a post crash fire ensued.

According to Air Traffic Control (ATC) communications, the airplane was cleared for departure from runway 22R: "Cherokee 9062 Juliet Chandler Tower, fly straight out runway 22 right, cleared for takeoff." One minute later, the pilot of another airplane reported the accident: "Hey Tower, I think that airplane that just took off in front of us just crashed." The controller reacted: "Cherokee 62 Juliet... Chandler Tower... you up?" There was no response from the accident pilot. Another pilot reported: "We're 10 miles to the southwest. We've got eyes on the fire and smoke."

The FAA reported: "Aircraft crashed under unknown circumstances and post crash fire."

The first identified point of impact was an impression in the dirt that lead to the main wreckage. The main wreckage consisted of the entire airplane. The left wing remained partially attached to the airframe and was impact damaged and consumed by fire. The right wing was impact damaged, separated, and consumed by fire and was located next to the airframe. The tail remained attached to the fuselage and remained free of extensive impact or fire damage. The main cabin area was consumed by fire. There was a bush that was pushed by the airplane from the dirt to the parking lot surface. The impact appears consistent with a low altitude aerodynamic stall/accelerated stall. The airplane most likely impacted terrain in a right wing low, nose level altitude.

KCHD features two runways, runway 4R/22L (4870 x 75 ft) and runway 4L/22R (4401 x 75 ft). The airport field elevation is 1243.4 ft.

- Weather:

The 1947 weather observation at KCHD included: Winds 230 at 7 knots, 10 miles visibility, temperature 39°C, dew point 16°C, and an altimeter setting of 29.74 inches of mercury. The calculated density altitude was 4567 ft.

On June 29, 2026, about 1945 eastern daylight time (EDT), a Diamond Aircraft Industries Inc. DA40 NG airplane, N602RF, was substantially damaged when it was involved in an accident near Petersburg, Virginia. The flight instructor sustained serious injuries, and the student pilot sustained minor injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight. 

The flight instructor reported that during the initial climb following a go-around on Runway 05 at Dinwiddie County Airport (PTB), the occupants heard a loud bang near midfield, followed by a reduction in engine power. The flight instructor assumed control of the airplane and observed that engine power decreased to below about 40%, resulting in a loss of expected climb performance; the engine continued to produce partial power. The airplane was unable to achieve sufficient climb performance to clear obstacles beyond the departure end of the runway environment. While assessing available areas for a forced landing, the flight instructor maneuvered the airplane, which resulted in activation of the stall warning horn. The flight instructor discontinued the turn and secured the engine by moving the fuel selector to the off position before the airplane impacted treetops. 

The airplane came to rest inverted in a densely wooded area about 1 mile northeast of the departure end of Runway 05, which resulted in substantial damage to the fuselage, wings, and empennage. 

A data card from the avionics installed in the airplane was retained and forwarded to the NTSB Recorders Laboratory for flight and engine data extraction. 

The wreckage has been recovered to a secure facility for further examination

On July 9, 2026, at about 2248 central daylight time, a Beech 95-B55, N202DK, was destroyed when it was involved in an accident near Waterloo, Illinois. The pilot and pilot rated passenger were fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight.

Family of the pilot reported that he had departed St. Louis Regional Airport (ALN) Alton/St. Louis, Illinois, on July 9 with an intended destination of Smith Field Airport (SLG), Siloam Springs, Arkansas. A family member, who was monitoring the airplane’s progress on a flight tracking application, saw the airplane stopped transmitting position data partway through the flight. The family member became concerned that the airplane may have crashed and subsequently notified local law enforcement. The FAA issued an Alert Notice (ALNOT) shortly after. The wreckage was located by Search and Rescue personnel the morning of July 10. There are no known witnesses to the accident sequence. Local law enforcement reported that on the night of the accident, the area experienced a significant storm with heavy rain, lightning, strong wind, and low visibility.

Recorded ADS-B data showed that at 2226:06, the airplane departed from ALN on a southeast heading and climbed to altitude of about 3,800 ft mean sea level (msl). At 2238:28 the flight track data showed the airplane made a right turn to a southern heading and climbed to an altitude of about 5,000 ft msl. At 2240:54 the airplane made a right turn to a southwest heading and maintained an altitude of about 5,000 ft msl. At 2246:08 the airplane made a right turn to a western heading and descended to an altitude of about 4,500 ft msl. At 2247:41 the airplane made a left turn to the south and descended to about 4,100 ft msl. At 2247:57, while on a southern heading, the airplane climbed to an altitude of about 5,600 ft msl and its ground speed slowed to about 22 knots. At 2248:13, the data showed that the airplane initiated a descent, which continued until ADS-B contact was lost about 1,000 ft northwest of the accident site at 2248:40.

Examination of the accident site revealed that airplane’s wreckage came to rest in a field bisected by a line of trees about 4 miles southeast of Waterloo, Illinois. The tree line was orientated in a north / south direction and was about 30 ft wide with tree heights between 45 and 60 ft. The first identifiable point of contact (FIPC) was a ground scar that was about 8 ft in length and 30 ft in width. The debris path was oriented on a true heading of about 130° and was about 330 ft in length. The debris path extended through the tree line into an open grass field adjacent to the FIPC. The main wreckage was located about 50 ft from the FIPC within the tree line. Throughout the debris path, various portions of wing fragments, fuselage fragments, left engine, propeller blades, propeller hub fragments, and ailerons were observed. Additionally, several trees were damaged throughout the debris path. All major structural components of the airplane were located at the accident site.
